The forms on this page can be printed, completed, and turned in to any Financial Services Campus nearest you. Many of these forms need to be filled out in addition to your FAFSA on the Web.



2008 - 2009 forms




Students who are denied financial aid may be eligible to submit an Academic Petition requesting consideration based on extenuating circumstances.


General BC demographics form. Including the permission to release financial aid information to other people.

The purpose of this agreement is to provide financial assistance to eligible students whose primary enrollment is at BC but who are also attending another institution at the same time. This form is also for students attending the BC/Sheridan automotive program. The form allows BC to count the credits at the other school for maximum financial aid.

The Loan Appointment is required if a student met/exceeded the BC loan limit of $14,125. This amount includes any loans from other institutions. Student must completely explain reason for excessive borrowing, and must include an education plan.

Form needed to process any loans for eligible students interested in borrowing.


The Master Promissory Note (MPN) authorizes the lender to disburse loans. The MPN is between the lender and the student and is necessary, upon request and upon the school's certification of loan eligibility.


The federal government has very specific criteria in considering a student to be independent.(see student status on FAFSA form) Self support in and of itself does not qualify a student to be independent. If there are extraordinary circumstances that you feel might change your dependency status, please read this form and complete all of it to be considered for a dependency override


If a family’s current income has been reduced by special circumstances, a financial aid administrator may be able to use the current income to assess financial need. Please provide information regarding your income reduction by completing this form and attaching the requested documentation.

Please fill out this form if you are canceling all or a portion of your loan for current year.

Application submitted by students who wish to be considered for scholarship funding.

Form to submit when a check was not received. Form must be filled out completely with a copy of student’s drivers’ license.

If you withdraw from or audit a class once the 100% refund date has passed you should file this form. If this withdrawal results in a decrease in enrollment you may be billed for aid received and/or be forced to forego any potential increase in aid for a class(es) that have not yet reached their 100% refund date.



2008 - 2009 work study forms

Employment packet for students interested in working on campus under the Federal Work Study Program.

 

Employment packet for students interested in working as student assistants working on campus; as well as off campus tutors in selected elementary schools under the Florida Work Experience Program (FWEP).

 

Form used for student interested in directly depositing their work study checks. This process is highly recommended because it reduces the time it takes to receive your pay check.

This form is needed to request work study students for the academic year.


Use this form to accept or decline your Federal Work Study award and return to your campus Student Financial Services office.

Form used for current and returning students who are applying for work study positions.

 

This form is used when requesting a work study student (s) for the academic year.

This form is used when requesting a Florida Work Experience Program (FWEP) work study student (s) for the academic year.

This form is used when terminating a student who is currently working for a department.
