Forms

The following forms (in WORD format) are used by the BC Curriculum Committee to transact/document college-wide curriculum proposals. BC and State of Florida information resources are available to assist you in completing these forms … check the navigation menu at left.

All completed forms (except  ‘Career and Technical Education New/Expansion of Program Procedure'  and  'F/T Faculty Endorsement of New GenEd Course Inclusion') must be transmitted to the Curriculum Services Office by e-mail attachment (or hand-deliver a disk with all completed files to the Curriculum Services Office: WHC, 31-309d), on or before the advertised proposal deadline. Please use the Curriculum Committee e-mail address  (currcomm@broward.edu)  to transmit completed proposal documents and e-acknowledgements.

Note (Program of Study):  If you are planning to revise an existing A.S., A.A.S., and/or certificate program of study, a  Current  and a  Proposed program of study sheet are required to be submitted with your proposal documents. For your convenience and accuracy, the '08-'09 program of study sheets can be found on this web site (A.S., A.A.S., Certif.). Submitted  Current  and  Proposed  program of study sheets must come from this web link.  A.A. program of study sheets may also be obtained at this web site (Program Sheets: A.A.).

PLEASE BE ADVISED:  Depending on the accuracy of the completed forms for your proposal, some or all of the paperwork may be returned to you due to lack of completeness and/or errors.  If necessary revisions to your proposal cannot be completed in a timely manner to process, post to the web site, and give proper agenda/meeting notice to Curriculum Committee members and BCC faculty/administrators, your proposal will be posted as new business to the subsequent month's agenda.  Please start your proposal drafting early and contact the Office of Curriculum Services (Contact Us), if necessary.

You will need to complete this form for any type of proposal, including FYI, to be presented to the Curriculum Committee. In the navigation menu on the left-side of the screen, you will find links to the Florida Department of Education (DOE) and the State Course Numbering System (SCNS ... under menu item DOE Links (Proposals)) to assist you in researching degree programs and course information for your proposal.

Since this form is the main document to be posted to the web site for public announcement of the proposal, please complete the form in its entirety and give as much detail as possible in sections 2 (Summary of Proposal) and 3 (Rationale for Proposal).

Waive  of  First  Read:  Regular committee procedure has the presenter/originator explain and answer questions about the proposal at the first meeting (first read) and return to the next meeting (following month) for follow-up questions and the committee’s vote on the proposal. By marking yes on ‘Request Waive of First Read’ on the articulation form, the presenter/originator is requesting the committee to vote on the proposal after the presentation at the first meeting. There will be 2 votes by the committee: 1. vote to accept the request to waive first read, and 2. vote on the curriculum proposal. Please see CC Procedure Manual (section E: General Guidelines) for further information on Waive of First Read.

Permanent  Course  Information  (PCI)  Form

With the State Course Numbering System (SCNS) having implemented a real-time, interactive inventory database of institutional/state courses, BC’s Office of Curriculum Services has been able to dispose of the state transmittal forms for new and change/termination of courses.

All the course attributes necessary to establish a new course (or modify an existing course) on CID and the SCNS inventory are now captured on BC’s PCI form. For NEW courses, please complete page 1 of the form; for course changes/terminations, please complete page 2.  Page 3 contains directions/definitions for pages 1 and 2.

Please complete this page for each NEW course(s) proposal only.

Please complete this page for each CHANGE(S)/TERMINATION(S) of an existing course.

This page contains DIRECTIONS/DEFINITIONS for pages 1 and 2.

Presenters/originators no longer need to produce a current PCI for existing courses and mark the appropriate changes in red. The Office of Curriculum Services will update the existing PCI based on the requested changes indicated on page two of the PCI form.

This form is used by BC to document all special fees that are charged for the course. If your proposed course(s) has any type of special fee assessment, you must complete this form for each course.

Please make sure that campus administration personnel that must approve all special fee amounts on courses, acknowledge: (1) the amount of the special fee and (2) campus approval of the special fee amount.

If you are proposing a NEW degree program (A.S., A.A.S., certificate, ATD), you will need to print a copy of this form and start the process of getting each part of the 3 steps initialed.  Please forward the initialed document by pony mail (or hand-deliver) to: Office of Curriculum Services, Bldg. 31-309d, WHC.  You may review a copy of the Career and Technical Education rationale for this procedure by clicking here.

NOTE: This form is also used for PROGRAM EXPANSION (i.e. offering an existing program at an additional campus site).

This form must be completed in its entirety for each NEW course proposed.  Due to the recently completed General Education review, the course outline template has been updated as of 10/3/06.  A sample of a completed course outline template may be viewed by clicking here.

NOTE:  If you are planning to propose a course for General Education status, you will need to complete the  General Education New Course Form  and  F/T Faculty Endorsement of GenEd Course Inclusion as part of the documentation for your proposal.  These documents will be forwarded to a General Education sub-committee for action; the collegewide Curriculum Committee does not act on general education requests within the proposal.

For EXISTING courses: 1. Call the Office of Curriculum Services (Contact Us) for a WORD format copy of the current course outline; 2. Submit modified course outline with appropriate deletions in strikethrough and additions in color.

This form must be completed for all NEW PSAV programs of study, and modifications to EXISTING PSAV programs of study. Per the instructions, please forward the completed form to the AVP for Technical Education (Ms. Matilde Roig); with an e-copy of the completed form being submitted to the Office of Curriculum Services, with your other completed curriculum proposal-related documents.
