Veterans Forms



All VA forms are e-forms available at the Veterans Online Application System (VONAPP) once
you log in through the GI Bill web site. Before submitting the electronic application,
please print and sign a copy, and submit
to any Student Financial Services Office.

1. First time applying for VA Education Benefits.

Please complete and submit an Application for VA Education Benefits VA Form 22-1990 to VA. Application is available for electronic submission online at: http://vabenefits.vba.va.gov/vonapp/main.asp. Print and submit a signed copy of the form along with the Broward College Veterans Certification Form (VCERT). Don't know what education benefits you are eligible for please visit the GIBILL website: http://www.gibill.va.gov/ or call the Veterans Administration at 1-888-442-4551.

    In addition, you may be required to provide one or more of the following documents when you submit your electronic application to VA.

    • A Copy of the DD form 214 member 4 copy (Chapters 30, 33 and 1607 only)
    • DD Form 2384, Notice of Basic Eligibility, NOBE (Chapter 1606 reserve/national guard with drilling time)

Transfer of Post-9/11 GI-Bill Benefits to Dependents (TEB) (CH33) - Any member of the Armed Forces (active duty or Selected Reserve, officer or enlisted) on or after August 1, 2009.

  • Upon approval from DOD, family members may apply to use transferred benefits with VA by completing VA Form 22-1990e. . Print and submit a signed copy of the form along with the Broward College Veterans Certification Form (VCERT) signed by an academic advisor.

2. Are you a Continuing Broward College Student?

YES - Submit the Broward College Veterans Certification Request Form (VCERT) signed by an academic advisor.

NO - Please complete and submit a Request of Change or Program or Place of training VA Form 22-1995. The form is available for electronic submission at: http://vabenefits.vba.va.gov/vonapp/main.asp . Print and submit a signed copy of the form along with the Broward College Veterans Certification Request Form (VCERT) signed by an academic advisor.

Have you changed your program of study since your last enrollment certification?

YES - Please complete and submit a Request of Change or Program VA Form 22-1995. The form is available for electronic submission at: http://vabenefits.vba.va.gov/vonapp/main.asp . Print and submit a signed copy of the form along with the Broward College Veterans Certification Request Form (VCERT) signed by an academic advisor.

Has it been over a year since your last enrollment certification/transferred schools?

YES - Please complete and submit a Request of Change or Program or Place of training VA Form 22-1995 to re-active your file. The form is available for electronic submission at: http://vabenefits.vba.va.gov/vonapp/main.asp. Print and submit a signed copy of the form submit along with the Broward College Veterans Certification Request Form (VCERT) signed by an academic advisor.

3. Dependent of Veteran (CH35): A veteran who died or is permanently and totally disabled as the result of a service-connected disability. The disability must arise out of active service in the Armed Forces.

  • First time using Dependent Benefits VA (CH35) education Benefits
    • Please complete VA Form 22-5490 - Print and submit a signed copy of the form along with the Broward College Veterans Certification Request Form (VCERT) signed by an academic advisor.
  • Changed your program of study since your last enrollment certification
  • Over a year since your last enrollment certification
  • Changed place of training
    • Please complete VA Form 22-5495 - Print and submit a signed copy of the form along with the Broward College Veterans Certification Request Form (VCERT) signed by an academic advisor.

Broward College Veterans Certification Request Form (VCERT) available at Broward College/ veteran's web page please print and complete and submit.
All students must meet academic progress standards please review the Standards of Academic Progress requirements on the Broward College Website.

All transcripts of previous training should be submitted within the first semester of attendance. Failure to do so may result in a suspension of VA educational benefits
