Workers Compensation Procedure

If you need emergency medical assistance, call 911 and Campus Safety.

 

If 911 emergency medical assistance is not required but medical care will be sought:

  1. Contact the Campus Safety Office.

  2. Complete an Accident-Incident Report with the Campus Safety Officer.

  3. Complete a FCC RMC Report with the Campus Safety Officer.

  4. If medical attention is required, the Campus Safety Officer will issue a referral to a WC facility. You must have a referral to an authorized WC facility before you can receive treatment for the injury. Campus Safety has a list of authorized facilities. You cannot go to your physician for care or it will not be covered under Workers Compensation.

  5. Notify your supervisor if you will be leaving campus to seek medical treatment for the injury.

  6. Go to an authorized WC facility for treatment within 24 hours of injury.

  7. After receiving treatment, a release will be given to you by the treating physician. You must fax and/or deliver the original release to your supervisor to determine when you will be required to report back to work. You must also submit a copy of the release to the Benefits Office via fax number (954) 201-7096.

  8. If the release states that you are unable to return to full-duty work immediately, you still must notify your supervisor and the Benefits Division at (954) 201-7337. If the release states that you may return to work full-duty on a specified date, you MUST report back to work on the date as indicated.
