Workers Compensation Procedure If you need emergency medical assistance, call 911 and Campus Safety. If 911 emergency medical assistance is not required but medical care will be sought: - Contact the Campus Safety Office.
- Complete an Accident-Incident Report with the Campus Safety Officer.
- Complete a FCC RMC Report with the Campus Safety Officer.
- If medical attention is required, the Campus Safety Officer will issue a referral to a WC facility. You must have a referral to an authorized WC facility before you can receive treatment for the injury. Campus Safety has a list of authorized facilities. You cannot go to your physician for care or it will not be covered under Workers Compensation.
- Notify your supervisor if you will be leaving campus to seek medical treatment for the injury.
- Go to an authorized WC facility for treatment within 24 hours of injury.
- After receiving treatment, a release will be given to you by the treating physician. You must fax and/or deliver the original release to your supervisor to determine when you will be required to report back to work. You must also submit a copy of the release to the Benefits Office via fax number (954) 201-7096.
- If the release states that you are unable to return to full-duty work immediately, you still must notify your supervisor and the Benefits Division at (954) 201-7337. If the release states that you may return to work full-duty on a specified date, you MUST report back to work on the date as indicated.
