The following Honors Institute forms (for Faculty) are available in Adobe Reader* and/or Microsoft Word format to download:

Honors Syllabus Template
This Honors Syllabus template was created so that all Honors course syllabi will have a unique and recognizable appearance to distinguish them from conventional courses. The template is a MS Word document containing a special header and footer. The header was designed to contain the generic information that all courses have in common (e.g. Title, reference #, term, time and classroom location, and so forth). Course specific information would be located in the body of the document and may be formatted according to personal preference. The footer contains contact information for the three Campus coordinators and the WHC collegewide offiice.

All Honors course syllabi should use this template.
If you wish, you may email your syllabus to the collegewide office to have the formatting completed for you.
 
Faculty Recommendation Form
Please use this form to refer students from your classes that you would like to recommend for enrollment in the Honors Institute.
Deadline: none
 
International Travel Award Application
Please see the Travel Award page and the form for further information.
Deadline: March 21, 2008
 
Honors by Contract
Students can use this form to apply for Honors credit for a non-Honors course. Please read the form for a detailed description and instructions.
Deadline: The student must fill out the form within the first 4 weeks of class.
 
Departmental Award Nomination Form
Use this form to nominate a student for a Departmental Award on your campus. Nominators do not have to present the award. The form includes a description of the award and the criteria.
Deadline: April 18th, 2008
 
Dean's Award Nomination Form
Use this form to nominate a student for a Dean's Award on your campus. Nominators do not have to present the award. The form includes a description of the award and the criteria.
Deadline: April 25th, 2008

2. Make a note of where you download the program on your computer.
