The following is a list of additional procedures you will encounter on a regular basis as an advisor or counselor.

Substitutions and Waivers |
Some courses may be substituted automatically in the student’s degree program.
Computer courses
For students who began here prior to the Fall of 2007 (20081) and have not broken ctalog year, the following may be substituted for CGS1061c:
CGS1000, CGS1570, CGS1100
Any other substitution must be applied for using the Course Substitution and Waiver Form.
Wellness courses
For students who began here prior to the Fall of 2007 (20081) and have not broken catalog yearm the following may be substituted for HSC1101c:
Any two (2) credit wellness course such as HLP1081, PEM1116, PEM1131, PEM1141, or PEM1181.
Any other substitution must be applied for using the course substitution waiver form.
1997 Transfer courses
Some evaluated transfer course work may appear with 1997 numbers which means the course is not identical to BC courses but may still apply to a general education area. Some of these courses can be added to the general education area.
Examples are:
HUM1997 Humanities
AMH1997 History
MAC1997 Math
BSC1997 Biology
CHM1997 Chemistry
Check with your coordinator before automatically substituting these courses. Other evaluated course work that has been unclassified (UNC1997) will apply to the elective area. You must always use the course substitution waiver form when requesting to use a UNC1997 course.
You should provide appropriate documents any time when using the course substitution waiver form (such as course descriptions and transcripts).
300 Level Course Substitutions
An upper division 300 or 400 level course may occasionally substitute for a lower level course that is equivalent or similar. Use the 300 Level Waiver Form and provide course descriptions and transcripts.
Repeat courses
A substitution may be requested to have a grade for one course replace the grade for another course, particularly when the student’s GPA is of concern. This is only when the transfer course is not identical to any course at BC. The repeat course must be similar to the transfer course (provide course descriptions and transcripts). Use the course substitution waiver form and check the “repeat” box.

SW SW).
When using the "Course substitution waiver" form, send the top (white) copy to Susan Desbrow, Graduations department, DTC, and file the remaining copies in the student's file folder.
Repeat waivers and 300 level waivers must be sent to the evaluators at the DTC. |

Petition Requests
Academic Standards Petitions
The Academic Standards Committee may grant exceptions of academic regulations. Students may initiate petitions with their advisor/ counselor, but the Dean of Student Affairs must sign petitions. Refer to the catalog for a description. Students may refer to the Student Life Handbook
The Academic Standards Committee hears appeals
from students on matters related to academic policies
such as standards of progress, graduation
requirements, and repeating courses. The Committee
makes recommendations to the Vice President for
Student Affairs in his/her role as College Student
Ombudsman. The following procedure shall apply to
requests for exceptions to established academic
policies:
1. The student shall complete the Academic
Standards Petition that is available at all
student affairs offices.

The Petition shall
include all pertinent and relevant
documentation such as transcripts, letters
from the transferring institution, medical
documentation. If the petition is a request
for admission while on suspension or
dismissal from another institution, the
student should include a letter of support (if
available) to attend Broward
College from the previously attended
institution.
2. The campus/center chief student affairs
officer or designee must sign the petition and
forward it to the College Registrar’s office no
later than one week prior to the scheduled
meeting. Exceptions must be approved
through the campus/center chief student
affairs officer.
3. The dates, places and times of the Academic
Standards Committee are published in the
College calendar and can be obtained from
the campus/center student affairs offices.
4. In cases involving entering or re-entering
Broward College after
suspension, the student shall be required to
attend the meeting. Students will appear
before the Committee in order of sign-in.
5. After careful review of the petitions, the
Committee shall make recommendations to
the Vice President for Student Affairs. The Vice President for Student Affairs approves
or disapproves the recommendations from
the Committee.
6. The student shall be notified in writing of the
Vice President’s decision. |
Application for Cancellation
of Previous Academic Record
Students in Associate in Science, Associate in
Applied Science, Certificate or Diploma Programs
who have previous unsatisfactory academic records
may petition for cancellation of their previous
academic record.
If, at the end of 24 credit hours,
Associate in Science and Associate in Applied
Science Degree students have achieved a grade
point average of 2.0 or above, they may be granted
cancellation of the previous unsatisfactory academic
record, except in the case of students enrolled during
or later than Term I, 1997-1998. For Certificate and
Diploma students, the requirement is completion of
one-third of the total program hours and a grade point
average of 2.0 or above. Normally, this cancellation
will not be approved unless the unsatisfactory student
performance is at least two academic years old.
Canceled academic records will be completely
disregarded in the calculation of credit hours and
grade point average. However, these students'
permanent records will show all work attempted and
all grades earned along with a notation about the
cancellation granted by the College.
Under the articulation agreement binding the Florida
public community colleges and state universities, this
policy may not be applied to Associate in Arts degree
students.

| Completion of Graduation Requirements After Transfer |
| In order
to graduate from Broward College, students must complete at
least
25% of their prescribed college-level coursework at BC and be enrolled
at BC during the term that the degree is completed.
If a BC student
has completed 25% of his or her coursework at BC but wishes to complete
the remainder of his or her coursework at another instituion and still
receive
his
or
her degree
from Broward College, he or she must petition the academic
standards
committee and be approved to do so prior to attending the other institution.
If said student is dual enrolled at BC and another institution
during the term in which degree requirements are completed, he or she
will still have residency at BC and does not have to receive prior
approval from Academic Standards.
It is important to note that students who have completed less than 25%
of their coursework at BC must also petition Academic standards in order
to graduate from BC, regardless of whether or not they wish to complete
degree requirments after transfer. Those are two seperate issues. A student
who has completed less than 25% of their prescribed coursework at BC
and wishes to complete degree requirements after transfer must submit
a seperate Academic Standards petition for each issue.

Petition for Exception to Full Cost Requirement
Students may petition for exemption from the full cost of tuition for the third attempt of any course. This petition originates from and must be approved by the Student Affairs Dean.

This form can be used by the advisor or counselor for course recommendations. Students should be given the carbon copy and the original should be filed as a record of the advisement session

This from originates in the registration department. Students should keep a copy of this form as a record of their withdrawal from classes.

BC Transcript Request
Students must use this form in order to request to have their BC transcripts sent to other institutions.

Student now have the option of requesting transcripts online by logging in to MY BC and then selecting the Transcripts option under "Quick Links" on the MY BC homepage. |
Transcript Request (to BCC from other institutions)
Students may use this form to request transcripts be sent to BC from other institutions. However, when possible, students should be encouraged to request electronic transcripts. Hand-carried transcripts can only be accepted at Registration if they are ‘official’ and ‘sealed’. Students attending institutions from other countries should be given the Josef Silny & Associates ‘Application for Evaluation of Foreign Educational Credentials’.

Students who wish to enroll in their college prep (remedial) courses at community adult centers must submit this completed form to the Registration department.

Testing Forms
Private Public Provider Remediation & CPT Payment
Students who have completed their college prep (remedial) courses outside of BC and who wish to retest must submit this form to the Testing Coordinator prior to testing. Students must also show receipt of payment.

Placement Test Retake Form
This is the Placement Test Retake form.Students are allowed to retake the placement test If 90 days or more have passed since the last test date

Computer Literacy Exam Referral Form
This is the Computer Literacy Exam Referral Form given to students to exempt from having to take Computer and Internet Literacy (CGS1060C) in programs that do not specifically require completion of CGS1060C.

Mathematics Requirement Verification
This form should be signed by the advisor/counselor and given to the student to submit with their nursing application (or any Health Science application if appropriate).

Transfer Work - Unofficial Evaluation
Students who wish to apply to any Health Science program, but whose transfer coursework has not been officially evaluated should use this form. Students should supply a copy of their transcripts.

This form is used to refer students to the counseling/advisement office or any other department.

Candidate for Graduation Form
Once application for graduation has been completed on the computer, this form should be given to the student. The student will then be able to pay their diploma fee at the cashier window.
