Florida Resident Form-This form is for new, returning or continuing students who wishes to declare initial Florida Residency classification or reclassification.
Non Florida Resident Form - This form is for new or returning students who do not qualify as Florida resident for tuition purposes.
Florida Classroom Teacher Tuition Fee Waiver - Fee waiver for teachers employed full time by a school district in the state of Florida.
Florida State Employee Tuition Fee Waiver - This form is for Florida state employees seeking a tuition fee waiver for courses taken at Broward College. Students seeking this waiver must complete a Broward College application for admission and complete all admission requirements. Registration begins for Florida state employees on the date listed on the registration dates calendar.
Homeless Exemption Certification - Waiver of tuition and fees payment for homeless students.
Appeal of Residency Denial - This form is for a student who wishes to appeal the denial of residency for tuition purposes. The student must seek classification/reclassification at the campus prior to submitting a request to the college-wide appeal committee.
Home School Completion Affidavit - A student who is claiming that home school graduation requirements are complete.
Please Note: The Home School Affidavit must be completed by the student's parent or legal guardian. (If the student is already logged into 'Broward College One Access,' he/she must log out.)
Name Change - A student whose legal name has changed may request an update of their student information. Legal supporting documents are required as verification of the change.
SSN Change - A student whose social security number has changed may request an update of their student information. Legal supporting documents are required as verification of the change.
Non Immigrant Guideline - Foreign nationals on a non immigrant visa classification may be eligible to attend, and may be eligible to establish Florida Residency for Tuition Purposes.
Sworn Affidavit of Family Ties - A student who claims family ties to Florida in support of Florida Residency classification.
Self-Certification of Educational Background - A student who cannot obtain transcripts from a prior institution due to extenuating circumstances (i.e. the school is permanently closed, the school's records have been destroyed as a result of a natural disaster, etc.). A personal statement explaining the extenuating circumstances and proof of the attempts to retrieve a copy of the transcripts from the Department of Education (if the school is in the United States) or the Ministry of Education (if the school is in a foreign country) must be submitted with this form.
Academic Standards Petition - A student who believes they have an extraordinary circumstance and seeking to appeal a College Policy.
Change of Program - A student who wishes to change their primary academic objective.
Change of Program for International Students - An international student (F1 visa) who wishes to change his/her primary academic objective.
Health Science Limited Access Application: This form is for students who have completed the admissions requirements and are eligible to apply for a Limited Access Health Science Program.
MAC1105C to MAT0022 Transfer Request - A student who would like to be dropped from a section of MAC1105C and enrolled in a section of MAT0022. This form is only valid during the second week of the term that the student is enrolled in MAC1105C.
Petition of Maximum Course Load - A student who wishes to enroll for more than 18 credit hours in a term.
Request for Evaluation of Military Courses - A student that wishes to transfer courses earned while in the military.
Transient Student Form – A student seeking a degree at Broward College who would like to take courses that apply to this degree at another post-secondary institution on a temporary basis.
Submit Petition for Refund Request- A student may submit a petition for refund for a withdrawal after the published 100% refund deadline only when non-academic extenuating circumstances prohibit a student’s ability to withdraw. Such non-academic circumstances may include, but are not limited to, serious illness of self or of a family member, death in the immediate family, and other emergency circumstances.

When you are selected for verification, Broward College requires certain documents to compare your FAFSA data with your financial and family information. If differences arise your financial aid awards will be delayed until all discrepancies are resolved.

Loans
Broward College participates in the Federal Direct Loan Program as well as the PLUS loan program for Parents. Learn more about student loans and the steps required to complete the loan process.
Effective July 1, 2012, Broward College policy allows only students in the categories listed below to request unsubsidized loans.
Parent PLUS loans are available to dependent students only; a dependent student is determined during the completion of the Free Application for Federal Student Aid (FAFSA). Dependent students must have already completed the Free Application for Federal Student Aid (FAFSA) prior to the parent being considered for a parent PLUS loan.

Unusual Enrollment History (UEH)
Students with a UEH red flag will need to submit one of the forms below to the financial aid office* for review and to determine if further documentation is needed before you can receive federal financial aid.
APPLICANTS
International Student Supplemental Application - This form is used by international student applicants (F1 or M1 visa) to provide personal information required during the application process to Broward College.
Financial Affidavit of Support - This form is used by international student applicants (F1 or M1 visa) to provide proof of financial support for the duration of their studies at Broward College. Applicants will state if they have a financial sponsor(s) or they are using personal funds. Applicants who have more than one source of financial sponsorship will need to submit a Financial Affidavit of Support for each sponsor.
Affidavit of Support for Living Expenses (THIS FORM IS OPTIONAL) - Provide this form only if the applicant will be living with a relative or friend who will be responsible for all living expenses (i.e. housing, transportation, food, etc.) for the duration of the student's studies. If this form is provided, the financial sponsor can deduct USD $12,500 from the total funds required for the student and USD $7,000 for each dependent, if applicable.
F1 Transfer Eligibility Form - This form does not guarantee acceptance nor transfer. It is used to determine if the student can be transferred to Broward College upon acceptance.
Application Status Student Release Form (OPTIONAL) - Student records are private. Students who wish to authorize someone to act on their behalf during the application process ONLY, including, but not limited to asking questions about the application status, are required to provide this form. The student’s signature must match the passport.
CURRENT STUDENTS
Change of Program for International Students - An international student (F1 visa) who wishes to change his/her primary academic objective.
I-20 Extension Verification - An international student (F1 visa) whose I-20 is expiring and needs additional courses to graduate.
Last Term to Graduate - An international student (F1 visa) who is in the last term to graduate and is seeking (1) an authorization for a reduced course load or (2) a confirmation of graduation term in order to apply for Optional Practical Training (OPT).
Opt-Out Credit Form: An international student (F1 visa) who wants to opt-out the commercial evaluation of foreign post-secondary transcripts. F1 students seeking a bachelor's degree are not eligible to opt-out credit.
Optional Practical Training (OPT) Employment Information: An international student (F1 visa) who needs to report his/her employment information, during pre or post-completion OPT.
HEALTH INSURANCE
Health Insurance Coverage Form: An international student (F1 visa) who needs to show proof of health insurance coverage.
Authorization for Release of Information (Enrollment Verification) - A student who seeks documented verification of their enrollment.
Duplicate/Replacement Diploma - A student who wishes to order a duplicate or replacement diploma that was previously earned.
FERPA Release - A student who wishes to give another person access to their records.
FERPA Directory Information Opt-Out Form - A student who wishes to “opt-out” of the release of directory information.
